Forms for Completion by Swimmers / Boat Captains / Crew Members - Submit no later than July 1st, 2017
Note: Boat Captains (Skippers) must complete the online boat safety checklist and declaration. Swimmers that register for the swim (with boat) must have their boat captain complete this online form. Swimmer’s please provide your boat captain with your email address to enter on the form and you will receive an automatic email notification once your boat has been registered. Boat Captains, please read the Galway Bay Swim Safety Document prior to completing the online form.

Note: Your signed and completed medical and waiver form can be scanned and emailed to David.ODonnell@cancercarewest.ie or Post to Galway Bay Swim, C/o Cancer Care West, Inis Aoibhinn Residence, University Hospital, Galway H91 T861, Ireland.
Note (1): The medical form must be completed within three months of the swim date.
Note (2): If you do NOT have a boat confirmed for the swim, then you can delay submitting your medical form.

Note: This online Form is only for swimmers non-resident in Ireland that cannot attend the swim trial scheduled in Galway. It is expected that swimmers resident in Ireland will attend the swim trial in Blackrock, Salthill, Galway in early June. Please check the Registration page for the exact date of this years 4km swim trial (2km for relay team members). The date of the trial will also be emailed to you when you register for the swim on Eventbrite.
