Forms for Completion by Swimmers / Boat Captains / Crew Members - Submit no later than July 1st, 2019
Note: Boat Captains must have insurance for their support boat. The insurance must have hull and liability cover, as per Safety Guidelines for Open Sea Swims (Swim Ireland). A copy of the Certificate of Insurance for the boat must be provided by the boat captain. It is the Swimmers responsibility to ensure that their boat captain completes this online form and uploads a photo or a scanned copy of the insurance certificate.
Swimmer’s please provide your boat captain with your email address to enter on the form and you will receive an automatic email notification once your boat has been registered. Boat Captains, please read the Galway Bay Swim Safety Document prior to completing the online form. Note that jet skis, lake boats or other boats that are NOT used regularly in the ocean are NOT approved for use on the Galway Bay Swim. Check our FAQ’s for more details.

Note: Your signed and completed medical and waiver form can be scanned and emailed to David.ODonnell@cancercarewest.ie or Post to Galway Bay Swim, C/o Cancer Care West, Inis Aoibhinn Residence, University Hospital, Galway H91 T861, Ireland.
Note (1): The medical form must be completed within three months of the swim date.
Note (2): If you do NOT have a boat confirmed for the swim, then you can delay submitting your medical form.
Note (3): Only Section B of the form needs to be submitted by the swimmer to the Galway Bay Swim. Section A (Medical History and Examination) must be retained by you and your doctor.
Note (4): Your medical form/email is destroyed or deleted after the swim each year. Cancer Care West are registered with the Irish Data Protection Commissioner.

Note: This online Form is only for swimmers non-resident in Ireland that cannot attend the swim trial scheduled in Galway. It is expected that swimmers resident in Ireland will attend the swim trial in Blackrock, Salthill, Galway in early June. Please check the Registration page for the exact date of this years 4km swim trial (2km for relay team members). The date of the trial will also be emailed to you when you register for the swim on Eventbrite.
